As a patient you have
the right to:
- be treated with dignity and respect in a non-discriminatory manner.
- know the names and professional titles of the people serving you.
- select and/or change your healthcare provider or request a second opinion.
- privacy during your visit.
- confidentially of your personal health information.
- participate with your provider in making informed decisions regarding your health care.
- consent to or refuse any care or treatment.
- review your medical record with a health care provider.
- voice concerns or complaints about services or policies.
As a patient you have
the responsibility to:
- provide complete and accurate information to the best of your ability about your health status.
- follow the prescribed treatment plan and ask questions if you don’t understand.
- report any significant changes in symptoms or failure to improve.
- show courtesy and respect to health care personnel and other patients.
- arrive for your appointment on time or cancel your appointment in advance.
- accept personal financial responsibility for any charges not covered by the health fee.
